Module 1 • Maternal Readiness

Female Pre-Pregnancy Health & Micronutrient Optimization

Optimizing maternal health before conception provides the developing embryo with an ideal biological environment from day one.

Pre-Conceptional Folic Acid: Begin taking 400 to 500 mcg of daily Folic Acid (or active L-methylfolate) at least 3 full months prior to attempting conception. This reduces the risk of neural tube defects (such as spina bifida and anencephaly) by up to 70%. Women with a BMI > 30, diabetes, or prior affected pregnancy require a higher 5 mg prescription dose.

Hormonal & Metabolic Screening: Screen fasting blood glucose, HbA1c, thyroid stimulating hormone (TSH), complete blood count (CBC), and blood grouping with Rh factor. Subclinical hypothyroidism (TSH > 2.5 mIU/L) should be corrected before conception to optimize implantation.

Medication & Skincare Audit: Immediately discontinue oral retinoids (isotretinoin), topical tretinoin, certain antihypertensives (ACE inhibitors/ARBs), and consult Dr. Berkheez Shabir to switch to pregnancy-safe alternatives.

Rubella & Hepatitis B Immunity: Check Rubella IgG immunity. If non-immune, receive the MMR vaccine and wait at least 4 weeks before attempting conception.

Module 2 • Paternal Health

Male Factor Health & Sperm Quality Optimization

Sperm spermatogenesis takes approximately 72 to 90 days. Lifestyle improvements made today directly elevate sperm motility, morphology, and DNA integrity 3 months from now.

Thermal Protection for Testes: The testes reside in the scrotum to remain 2°C to 3°C cooler than core body temperature. Prolonged heat exposure damages sperm DNA. Avoid frequent hot baths, sauna sessions, placing hot laptops directly on laps, and tight synthetic undergarments.

Antioxidants & Micronutrient Support: Sperm cell membranes are rich in polyunsaturated fatty acids that are highly vulnerable to oxidative stress. Daily antioxidant supplementation with Zinc (15-20 mg), CoQ10, L-Carnitine, Vitamin C, and Selenium supports progressive motility.

Smoking, Hookah & Alcohol Cessation: Tobacco smoke, vaping, and traditional hookah introduce polycyclic aromatic hydrocarbons and heavy metals that dramatically increase sperm DNA fragmentation, impairing fertilization and increasing early miscarriage rates.

Optimizing BMI: Central abdominal obesity elevates peripheral aromatase activity, converting testosterone into estrogen and reducing sperm count.

Module 3 • Timing & Conception

Cycle Charting & The 5-Day Fertile Window Matrix

Understanding your fertile window ensures sperm are already viable in the fallopian tubes when ovulation occurs.

Identifying the 5-Day Window: The fertile window encompasses the 5 days preceding ovulation plus the day of ovulation itself. The highest conception rates occur with intercourse 1 to 2 days before ovulation.

Frequency of Intercourse: Intercourse every 1 to 2 days during the fertile window maintains optimal sperm reserves. Daily intercourse does not deplete sperm count in healthy men, but every other day is equally effective and reduces psychological stress.

Lubricant Warning: Most commercial sexual lubricants (including saliva and petroleum jelly) are cytotoxic to sperm and impair progressive motility. Use sperm-friendly, hydroxyethylcellulose-based lubricants if needed.

Module 4 • Consultation Timeline

When to Schedule a Clinic Evaluation

Do not wait indefinitely if conception does not happen immediately.

Under 35 years: Seek evaluation after 12 months of regular unprotected intercourse.

35 years and older: Seek evaluation after 6 months of timed intercourse.

Immediate evaluation: Consult promptly if cycles are irregular (> 35 days apart), history of pelvic inflammatory disease, known endometriosis, or prior testicular surgery.

Consultations are held Monday to Saturday: Morning sessions at Bachan IVF Centre (Lal Chowk) and afternoon sessions at Apollo Clinic (Karan Nagar).

Patient Q&A

Frequently Asked Patient Questions

Should the male partner also attend the first pre-conception visit?

Yes, highly recommended. Conception is a shared biological process. In 50% of couples experiencing delays, male factors contribute. Dr. Berkheez Shabir evaluates both partners together to create a cohesive pre-conception roadmap.

How do I know if my menstrual cycle is irregular?

A normal menstrual cycle ranges between 24 and 35 days. If your cycle varies by more than 7 to 9 days from month to month, or if periods occur less frequently than every 35 days, you may have anovulatory cycles requiring clinical evaluation.
